Taking care of your health and well-being is a lifelong journey. As a member of an Aetna Dual Eligible Special Needs Plan, or D-SNP, you don’t have to go through it alone. You have an entire Aetna care team on your side every step of the way.

Your care team may include a nurse care manager, care coordinator, social worker, member advocate and health survey specialist. They are there to help you manage your health and get the most out of your benefits.

Soon after your plan begins, a member of your care team will give you a call to complete your health survey. This will help them get to know you and your health and wellness needs. And once it’s complete, you’ll receive a $50 gift card in the mail. Since your needs can change over time, the survey should be updated each year — your care team will reach out each year when it’s time.

Are you managing one or more health conditions? Struggling with certain symptoms? Confused about your medications? Sharing your questions and concerns with your care team can help them develop a care plan that best fits your needs.

That might include questions and concerns about seeing more than one health care provider. But don’t worry. Your care team may be able to help coordinate and schedule visits to your doctors, the dentist and other providers you need to see.

Having trouble getting to an appointment? Your care team may be able to help with that too. Most plans include transportation to and from approved locations. These include doctor’s offices and hospitals, pharmacies, senior centers and more.

Many D-SNP members have questions about their medications, like: How often should I take it? What are the side effects? What happens if I miss a dose? Understanding your medications is important for staying well. And your care team can help provide answers.

Beyond health care services, you also need a safe place to live to stay well. If that’s missing from your life, your care team can provide assistance. They can connect you with local and state programs that can help you with safe housing, your utility bills and more.

Sometimes, understanding your Medicaid benefits can be hard. If you have questions, your care team is there to answer them. They can help you access your state Medicaid benefits and make sure you get the benefits and services you need.
